DUI Service Provider Training - Registration

To Register

Class size is limited, so you must register to guarantee a place in the workshop/s of your choice. You may register by telephone with credit card information by calling Jeri at 217-206-7985 or Donna at 217-206-6050.

By mail - Complete the registration form provided and mail it with your payment to University of Illinois at Springfield, Institute for Legal, Legislative and Policy Studies, One University Plaza - MS PAC 451, Springfield, IL 62703, Attention: Jeri Frederick. Please include your DHS/DASA license number and/or other identifying number.

Please note that certificates of workshop completion will not be provided until payment is received.

Cancellation

You may cancel your registration and receive a full refund of payment, provided you notify UIS at least ten working days before the program date. We will refund your registration fee or alternately, credit the full fee to another of our training programs.

Withdrawals on or after the opening date of a program are subject to payment of the total fee. Of course substitutions are always welcome. "No shows" are non-refundable unless arrangements have been made in advance with UIS.

UIS must reserve the right to cancel scheduled workshops when insufficient enrollment is generated. In such an event, full refunds will be made to those registered for the canceled program, or at the registrant's request, fees may be applied to other UIS-sponsored programs.

Registration Confirmation

We will confirm your registration in writing approximately two weeks before the workshop date and send you information about the course location. Please remember to make hotel reservations early. You are responsible for your own overnight accommodations.

Inclement Weather Policy

In cases of inclement weather on training days, please call the training site to see if the program is cancelled for that day. This policy applies for those hopefully rare emergency conditions that make driving and travel extremely hazardous.
